Calcium and Elvitegravir

Here is the interaction we found between these two, with the evidence behind it.

Major

ELVITEGRAVIR (Vitekta)

Calcium seems to reduce levels of elvitegravir.
Advise patients to take elvitegravir either 2 hours before or 2 hours after taking calcium supplements. Pharmacokinetic research suggests that taking calcium along with elvitegravir can reduce blood levels of elvitegravir through chelation.
Severity Major Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the practical issue when calcium and elvitegravir (Vitekta), an HIV medicine, meet in your stomach. Calcium can bind to elvitegravir in a process called chelation, basically grabbing onto it so less of the drug gets absorbed into your bloodstream. That means lower elvitegravir levels, and with HIV treatment you really want steady, reliable levels so the virus stays controlled and doesn't get a chance to develop resistance.

The good news is this is easy to manage by spacing them out. The standard advice is to take your elvitegravir either 2 hours before or 2 hours after any calcium supplement. This isn't usually about food amounts of calcium, it's the concentrated supplement doses that matter most. Please confirm the timing with your pharmacist or doctor before changing anything.

About Calcium

What is Calcium?

  • Bone health and osteoporosis prevention
  • Dietary calcium deficiency
  • Heartburn relief (calcium carbonate antacids)
  • Supporting healthy teeth
  • High blood pressure support during pregnancy

Calcium is an essential mineral your body needs for strong bones, nerve signaling, and muscle function, and supplements can help fill gaps when diet falls short. Most people do best getting calcium from food, and supplements should be used at the lowest dose needed. Talk to your pharmacist or doctor before starting, especially if you have kidney problems or take other medicines.
