Drospirenone, Estetrol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Danshen interacts with.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the gist for Nextstellis (drospirenone with estetrol). The concern on file is that Danshen might block a liver enzyme called CYP1A2, which helps process certain medicines. In theory, slowing that enzyme could let more of your birth control hormones linger in your body, which might nudge up side effects like breast tenderness, nausea, headaches, mood changes, or spotting.

That said, I want to be honest about how thin this evidence is. It comes only from lab (test tube) studies, and when researchers actually tested Danshen in people, it did not meaningfully change a CYP1A2 drug. So a real-world problem here is unlikely. Danshen can also affect blood clotting, which is worth mentioning since the pill itself carries clot risk. Please run this by your pharmacist or doctor before combining them.

Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for your birth control pill Nextstellis (drospirenone and estetrol). These hormones are partly handled by a transporter in your body called p-glycoprotein, which helps move drugs in and out of cells. Danshen may slow that transporter down a bit. In a small study, danshen raised levels of another p-glycoprotein drug (fexofenadine) by roughly a third, so in theory it could nudge your hormone levels a little higher.

If that happened, you might notice more of the usual side effects, like nausea, breast tenderness, headache, or mood changes. The evidence here is fairly limited, and this is not a dangerous combination, but I'd still mention danshen to your pharmacist or doctor before starting it, just to be safe.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Your birth control pill, Nextstellis, contains drospirenone and estetrol, and both hormones rely partly on the CYP3A4 enzyme to be processed by your body. The concern here is that taking Danshen for a stretch of time (about 10 to 14 days in one small study) seems to speed up this enzyme, which could clear these hormones faster and lower their levels. In plain terms, your pill might not work as reliably, raising the chance of breakthrough bleeding, spotting, or even reduced contraceptive protection.

Keep in mind this evidence is limited and comes from studies using a different drug (midazolam), not your pill specifically. Interestingly, a single dose of Danshen had the opposite effect. Since pregnancy prevention is at stake, please check with your pharmacist or doctor before combining the two.
