Modafinil falls under P-GLYCOPROTEIN SUBSTRATES — the group Danshen interacts with.

Danshen might alter the levels of drugs cleared by p-glycoprotein.
Pharmacokinetic research in healthy volunteers suggests that danshen might affect p-glycoprotein activity. Taking danshen extract 1 gram three times daily for 10 days prior to receiving fexofenadine 60 mg increases peak levels of fexofenadine, a p-glycoprotein substrate, by 27.4% and area under the curve (AUC) by 37.2%.

Here's the gist for modafinil (Provigil) and danshen. Danshen seems to slow down a transporter in your gut called P-glycoprotein, which acts like a doorway that pumps certain drugs back out. When that doorway works slower, a little more of the drug can stay in your body. In a small study, danshen raised levels of another P-gp drug by roughly a third.

So in theory, danshen could nudge modafinil levels up a bit, which might mean stronger or longer effects like trouble sleeping, headache, jitteriness, a faster heartbeat, or anxiety. The evidence here is fairly limited and modafinil isn't a prodrug, so this isn't a guaranteed problem. Still, before combining them, check with your pharmacist or doctor.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the thing with Danshen and Modafinil: your body partly clears modafinil using a liver and gut enzyme called CYP3A4. Modafinil is the active drug itself (not a prodrug that needs switching on), so anything that speeds up or slows down that enzyme can shift how much stays in your system.

The research here is a bit of a mixed bag. When Danshen is taken steadily for a couple of weeks, it seems to speed up CYP3A4, which could lower modafinil levels and make it work less well, meaning you might feel sleepier than expected. But a single dose did the opposite and raised levels. Because of this push-pull, effects are hard to predict. The evidence is only moderate and based on a different drug, so please check with your pharmacist or doctor before combining them.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
