Modafinil falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of P-glycoprotein substrates.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.

Here's the gist with Rhodiola and modafinil. Your body uses a kind of cellular pump called P-glycoprotein to help move certain drugs around and out. In lab dish experiments, Rhodiola seemed to slow that pump down. If that happened in real people, a little more modafinil could stay in your system, which in theory might nudge up its usual effects, things like trouble sleeping, headache, jitteriness, a faster heartbeat, or feeling anxious.

I want to be honest about how soft this is, though. This concern comes only from test-tube research, not from studies in actual patients, so we don't know if it matters day to day. Modafinil isn't a prodrug, so the direction holds: more pump-blocking could mean slightly more drug. Before combining them, check with your pharmacist or doctor.

Modafinil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Here's the gist for Modafinil: your body partly clears this medicine using a liver enzyme called CYP3A4. In a test tube, rhodiola seems to slow that enzyme down a bit. If that happened in real life, modafinil might hang around a little longer and feel slightly stronger, which could mean more of its usual effects like trouble sleeping, headache, nervousness, a racing heart, or jitteriness.

That said, I'd keep this in perspective. This is mostly theoretical, and the effect varies a lot from product to product. In fact, a human study found that rhodiola did not meaningfully slow CYP3A4. So any real-world impact is likely minor. Still, since modafinil already affects sleep and alertness, it's worth a quick check with your pharmacist or doctor before combining them.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
