Why this applies to Testosterone
Testosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for combining Rhodiola with your testosterone. Your body partly clears testosterone using a liver enzyme called CYP3A4. In test-tube studies, Rhodiola can slightly slow that enzyme down, which in theory could let a little more testosterone hang around. If that happened, you might notice slightly stronger testosterone-type effects like acne, oily skin, mood changes, or fluid retention.
That said, I want to keep this in perspective. The evidence here is weak and mostly from lab work, and when researchers actually tested Rhodiola in healthy men using a CYP3A4 drug, it did not meaningfully change how the drug was processed. So this is rated a minor, mostly theoretical concern. Still, it's worth mentioning to your pharmacist or doctor so they can keep an eye on things.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. Testosterone replacement therapy is used in adult males whose bodies do not produce enough of their own testosterone. This includes primary hypogonadism — where the testes themselves fail due to causes such as Klinefelter's syndrome, injury, chemotherapy, or surgical removal — and hypogonadotropic hypogonadism, where the brain's signaling system (the hypothalamus or pituitary gland) fails to trigger testosterone production.
Testopel (subcutaneous pellets) may also be used to stimulate puberty in carefully selected males with clearly delayed puberty. The safety and efficacy of testosterone for age-related (late-onset) hypogonadism have not been established for most products, including AndroGel, Testim, Jatenzo, Kyzatrex, Natesto, Depo-Testosterone, Testosterone Cypionate, and Xyosted.
