Why this applies to Bendamustine

Bendamustine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.

Here's the gist for bendamustine (Treanda, Belrapzo, Vivimusta). Part of how your body processes this chemo drug runs through a liver enzyme called CYP1A2. In test-tube studies, rhodiola can slow that enzyme down a little, and the idea is that slowing it could let bendamustine build up slightly, which in theory might mean more of its usual side effects like low blood counts, nausea, tiredness, or higher infection risk.

That said, the evidence here is thin and mostly theoretical. A human study actually found rhodiola didn't change how the body cleared caffeine, another CYP1A2 drug, so any real-world effect may be small or none. Still, bendamustine is serious chemotherapy, so please don't add rhodiola without first clearing it with your oncology team or pharmacist.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. What Is Bendamustine Injection Used For?

Brand names: Treanda, Belrapzo, Vivimusta

Bendamustine hydrochloride injection is used to treat adults with chronic lymphocytic leukemia (CLL). It is also used for adults with indolent B-cell non-Hodgkin lymphoma (NHL) — a slow-growing type of blood cancer — when the disease has continued to progress during or within six months of treatment with rituximab or a rituximab-containing regimen.

For CLL, how well bendamustine works compared to other first-line treatments (aside from chlorambucil) has not been fully established.
