Why this applies to Thiotepa

Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Here's the short version: in lab dishes, rhodiola can slow down a liver enzyme called CYP3A4. Thiotepa is actually changed by CYP3A4 into a more active form called TEPA, so this one is a little tricky. If rhodiola really did block that enzyme in your body, it could in theory shift how much active drug you make, and thiotepa already carries serious risks like low blood counts, infection, nausea, and mouth sores.

The good news is the evidence here is weak and theoretical. A human study showed rhodiola did not meaningfully change another CYP3A4 drug, so real-world effects look small. Still, since thiotepa is powerful chemotherapy, please don't add rhodiola without first clearing it with your oncology team.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
