CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Clopidogrel falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with clopidogrel (Plavix): it's a prodrug, meaning it shows up inactive and your body has to switch it on before it can keep your platelets from clumping. That activation leans mostly on a liver enzyme called CYP2C19, not the CYP2C9 enzyme this Rhodiola interaction is about. So the file's concern (Rhodiola possibly slowing CYP2C9) doesn't line up neatly with how Plavix actually works.
The evidence here is also on the soft side, mostly lab data plus one small study in healthy men. For most people, short-term Rhodiola is unlikely to meaningfully change how well your Plavix protects you. Still, since Plavix guards against clots after a stent, stroke, or heart attack, I'd run any Rhodiola product past your pharmacist or doctor first.

However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with clopidogrel (Plavix): it's a prodrug, meaning it's swallowed in an inactive form and your liver has to switch it on before it can thin your blood. The enzyme that does most of that activating is CYP2C19, not the CYP1A2 enzyme this rhodiola interaction is about. So the lab-based concern that rhodiola might slow CYP1A2 doesn't line up neatly with how Plavix actually works.
On top of that, the evidence here is weak and mostly theoretical. One human study found rhodiola didn't meaningfully block CYP1A2 at all. I wouldn't lose sleep over this combination, but since Plavix protects you from clots, strokes, and heart attacks, it's worth a quick chat with your pharmacist or doctor before adding rhodiola.

In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing about clopidogrel (Plavix): it's a prodrug. That means it's inactive when you swallow it, and your body has to switch it on using liver enzymes before it can thin your blood and prevent clots. CYP3A4 is one of the enzymes that helps with that activation step.
On paper, Rhodiola might block CYP3A4, which could in theory slow how much active clopidogrel you make and give you slightly less clot protection. But I want to keep this in perspective. This is a minor, mostly theoretical concern based on lab studies, and a human study actually found Rhodiola didn't meaningfully change how the body handled a similar CYP3A4 drug. Because clopidogrel matters so much for your heart, just let your pharmacist or doctor know before adding Rhodiola.

What Is Clopidogrel Used For?
Brand name: Plavix
Clopidogrel tablets are used to reduce the risk of heart attack and stroke in people with acute coronary syndrome (ACS) — including unstable angina and non-ST-elevation or ST-elevation heart attacks — and in people with a history of recent heart attack, recent stroke, or established peripheral arterial disease. For ACS, it is typically given alongside aspirin.
It works by preventing platelets (blood cells that trigger clotting) from clumping together, which helps keep arteries open and lowers the chance of a dangerous clot forming.
