Why this applies to Trimipramine

Trimipramine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.

Here's the gist: trimipramine (Surmontil) is partly cleared by a liver enzyme called CYP2C9, and Rhodiola may gently slow that enzyme down. If it does, a bit more trimipramine could linger in your body. That could nudge up the usual tricyclic side effects, things like drowsiness, dry mouth, constipation, blurry vision, dizziness when you stand up, or a faster heartbeat.

That said, I don't want to overstate this. The evidence is mostly lab-based, plus one small study where Rhodiola slowed another drug only modestly. The effect also seems to vary a lot between products. So this is a be aware, not a be alarmed. Before adding Rhodiola, please run it by your pharmacist or doctor so they can watch for any extra side effects.

Theoretically, rhodiola might increase the risk of adverse effects when taken with antidepressants.
A review of adverse event reports in Poland identified cases of tachyarrhythmias, myalgia, arthralgia, gum pain, restless leg syndrome, swallowing disorders, and changes in consciousness when rhodiola was taken in combination with paroxetine, escitalopram, fluoxetine, sertraline, trazodone, and/or duloxetine.

If you're taking Trimipramine (a tricyclic antidepressant) and thinking about adding Rhodiola, here's the honest picture. The concern is mostly theoretical, and the evidence is anecdotal, drawn from a handful of case reports where rhodiola was paired with other antidepressants, not specifically with trimipramine. The worry is that stacking two things that nudge brain chemicals could add up and increase the chance of side effects.

With trimipramine, watch for things like a fast or irregular heartbeat, drowsiness, dizziness, dry mouth, or feeling "off" or restless. None of this is a sure thing, and it's rated as minor. Still, before you combine them, please run it by your pharmacist or doctor so they can look at your full medication list.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Trimipramine Used For?

Brand names: Surmontil, Trimip, Rhotrimine, Tripramine, Trimp

Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.

In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
