Rhodiola and Dexamethasone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Rhodiola interacts with whole categories of medication. Dexamethasone falls into 3 of those categories, so it can interact with Rhodiola in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

P-GLYCOPROTEIN SUBSTRATES

1 of 3

Why this applies to Dexamethasone

Dexamethasone falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of P-glycoprotein substrates.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the idea behind this pairing. Your body uses a little cellular pump called P-glycoprotein to move certain drugs, including dexamethasone, out of cells and back out of the body. In lab dish studies, rhodiola seemed to slow that pump down. If that happened in people, slightly more dexamethasone could stay around, which might nudge up the usual steroid side effects like trouble sleeping, mood changes, a bump in blood sugar, fluid retention, or stomach upset.

The honest part: this is theoretical, based only on test-tube work, not real patient cases, so the actual risk looks low. Still, if you take dexamethasone (especially long-term or for something serious), it's worth a quick chat with your pharmacist or doctor before adding rhodiola. Don't change your steroid dose on your own.

Moderate

IMMUNOSUPPRESSANTS

2 of 3

Dexamethasone also falls under IMMUNOSUPPRESSANTS.

Theoretically, rhodiola use might interfere with immunosuppressive therapy.
In vitro and animal research show that rhodiola has immunostimulatory effects.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS

Here's the idea with Rhodiola and dexamethasone. One of the jobs dexamethasone does is calm down an overactive immune system, which is why it's used for severe allergies, certain cancers, and inflammatory conditions. Rhodiola, on the other hand, has shown signs of revving the immune system up in lab and animal studies. In theory, those two could work against each other, making the steroid a little less effective at doing its job.

I want to be honest, though: this is a theoretical concern based only on test-tube and animal data, not human studies, so we don't really know if it matters in real life. If dexamethasone was prescribed to suppress your immune system, I'd skip Rhodiola and check with your doctor or pharmacist first.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Dexamethasone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: your body uses an enzyme called CYP3A4 to help break down and clear dexamethasone. In test-tube studies, Rhodiola can block that enzyme a bit, which in theory could let dexamethasone linger and build up. If that happened, you might notice more of the usual steroid effects, things like trouble sleeping, mood changes, a bump in blood sugar, fluid retention, or stomach upset.

The good news is this is mostly theoretical. The effect varies a lot by product, and a human study found Rhodiola did not actually slow the clearance of a similar CYP3A4 drug. So for most people this is a minor, low-probability concern. Still, since dexamethasone is potent, run the combo by your pharmacist or doctor first.

About Rhodiola

What is Rhodiola?

Rhodiola
  • Fatigue and low energy
  • Stress and coping (adaptogen)
  • Mental performance and focus
  • Mild depression
  • Physical and exercise performance

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

About Dexamethasone

What Is Dexamethasone Used For?

Brand names: Decadron, Hexadrol, Dexone, Ozurdex, Dexycu, Hemady, Dextenza

Dexamethasone is used to treat a wide range of conditions depending on the form and route. Oral tablets and solutions treat severe allergic reactions (such as asthma, contact dermatitis, and seasonal allergic rhinitis), inflammatory skin diseases, endocrine disorders including adrenal insufficiency and congenital adrenal hyperplasia, gastrointestinal diseases such as ulcerative colitis and regional enteritis, blood disorders, rheumatic conditions, kidney disease, and respiratory conditions including tuberculosis complications. Hemady (oral tablet) is specifically approved, in combination with other medicines, for multiple myeloma in adults. TaperDex (6-day and 12-day oral tablet packs) are used for the same broad range of allergic, inflammatory, and endocrine conditions.

Injectable forms (intramuscular and intravenous) are used when oral treatment is not possible, covering endocrine emergencies, shock, rheumatic disorders, and more. Eye-specific products treat different conditions: Dextenza (intracanalicular insert) treats eye inflammation and pain after eye surgery and itching from allergic conjunctivitis. Ozurdex (intravitreal implant) treats macular edema from retinal vein occlusion and diabetic macular edema, and non-infectious uveitis in the back of the eye. Dexycu (intraocular injection) treats inflammation after eye surgery. Maxidex (ophthalmic suspension) and dexamethasone sodium phosphate eye drops treat steroid-responsive inflammatory conditions of the eye. The ophthalmic/otic solution also treats steroid-responsive inflammation of the outer ear canal. A topical ointment (Seacall) is used for skin conditions including contact dermatitis, eczema, and neurodermatitis.
