CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses a liver enzyme called CYP1A2 to help clear out nortriptyline. In lab studies, rhodiola can slow that enzyme down a little. If that happened in real life, nortriptyline could build up more than usual, which might mean stronger side effects like dry mouth, constipation, drowsiness, blurry vision, or a faster heartbeat. Nortriptyline is an active drug (not a prodrug), so more of it in your system means more effect.
That said, this concern is mostly theoretical. A human study found rhodiola didn't actually change how the body handled another CYP1A2 drug. So the risk looks low, but since everyone reacts differently, just give your pharmacist or doctor a heads-up before combining them.

A review of adverse event reports in Poland identified cases of tachyarrhythmias, myalgia, arthralgia, gum pain, restless leg syndrome, swallowing disorders, and changes in consciousness when rhodiola was taken in combination with paroxetine, escitalopram, fluoxetine, sertraline, trazodone, and/or duloxetine.

If you're thinking about pairing rhodiola with your nortriptyline, here's the honest picture. Nortriptyline is a tricyclic antidepressant that already nudges up brain chemicals like serotonin and norepinephrine. Rhodiola may gently do something similar, so taking both together could add up and slightly raise your chance of side effects. With nortriptyline that might look like a faster or irregular heartbeat, dizziness, restlessness, or feeling unusually drowsy or foggy.
I want to be straight with you, though: this is rated as a minor concern and rests only on scattered anecdotal reports, not solid studies. It's a theoretical caution, not a red alarm. Still, before adding rhodiola, please check with your pharmacist or doctor so they can weigh it for your specific situation.
