Propranolol Hydrochloride falls under ANTIHYPERTENSIVE DRUGS — the group Rhodiola interacts with.

Theoretically, taking rhodiola with antihypertensive drugs might increase the risk of hypotension.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.

Here's the gist: propranolol already lowers blood pressure and slows the heart, and some early lab and animal research hints that rhodiola might nudge blood pressure down too. Put them together and, in theory, the blood-pressure-lowering effects could stack up a bit. In a baby being treated for a hemangioma, that might show up as unusual sleepiness, poor feeding, a slower heartbeat, or coolness and paleness from low blood pressure.

I want to be honest about the strength of this, though. This interaction is theoretical and comes only from test-tube and animal studies, not from real patients, so it may not amount to much in practice. Rhodiola also isn't intended for infants. Before giving your child anything alongside propranolol, please check with their pediatrician or pharmacist first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Propranolol Hydrochloride also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.

Here's the gist for propranolol: your liver uses an enzyme called CYP1A2 to help clear it from your body. In lab dishes, rhodiola can slow that enzyme down a bit. In theory, that could let a little more propranolol build up than usual, which might mean a slightly lower heart rate, lower blood pressure, or more sluggishness or tiredness. Propranolol is not a prodrug, so more of it means a stronger effect, not a weaker one.

That said, the real-world evidence is weak. A human study found rhodiola didn't actually change how the body handled a similar CYP1A2 medication, and the effect seems to vary a lot by product. Since this oral propranolol is usually given to infants for hemangiomas, please run any rhodiola use past your pediatrician or pharmacist first.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. What Is Propranolol (Infantile Hemangioma) Used For?

Brand name: Hemangeol

This intravenous (IV) form of propranolol is used in hospital settings to treat serious, life-threatening heart rhythm problems. It works on several types of abnormal rhythms, including those that happen during anesthesia or are caused by drug toxicity.

Specific uses include fast heart rhythms coming from the upper chambers of the heart (supraventricular tachycardia), dangerous rhythms caused by too much digitalis in the body, and certain fast rhythms triggered by excess stress hormones (catecholamines) during surgery.
