Why this applies to Pazopanib

Pazopanib falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of P-glycoprotein substrates.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.

Here's the gist for your situation: pazopanib (Votrient) is partly moved around and out of your body by a transporter pump called P-glycoprotein. Lab studies suggest rhodiola can block that pump. If it does, less pazopanib gets carried out, so more could linger in your system and push its effects higher than intended.

With pazopanib, that could mean more of its known side effects, things like high blood pressure, diarrhea, fatigue, nausea, or changes in liver tests. I want to be honest, though: this concern comes only from test-tube research, not from people, so it's more of a theoretical caution than a proven problem. Because pazopanib is a serious cancer drug, please don't add rhodiola without first checking with your oncologist or pharmacist.

Here's the thing to keep in mind with this pairing. The concern on file is that Rhodiola may have immune-stimulating effects, which could in theory work against medicines that are meant to calm or suppress the immune system. Pazopanib (Votrient) is grouped here as an immunosuppressant, but it's really a targeted cancer drug that slows the growth of kidney cancer cells. So this particular worry is mostly theoretical and based only on lab and animal studies, not on people taking Pazopanib.

That said, anytime you're on a serious cancer therapy, I'd rather you not add an herb like Rhodiola on your own. We just don't have good human data here. Please run it by your oncologist or pharmacist first so they can weigh in on your specific treatment plan.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Pazopanib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Your body clears pazopanib using a liver enzyme called CYP3A4. In lab (test tube) studies, rhodiola has been shown to slow that same enzyme down a bit. The worry on paper is that if rhodiola blocks CYP3A4, pazopanib could build up in your system, which might mean more of its known side effects like high blood pressure, diarrhea, nausea, fatigue, or liver irritation.

The good news is that this is mostly theoretical. A real human study found that a standard rhodiola dose did not actually slow the breakdown of another CYP3A4 drug. Still, since pazopanib is a serious cancer medicine, I would not pair it with rhodiola on your own. Please check with your oncologist or pharmacist first.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
