Clomipramine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP2C9.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.

Here's the gist: lab studies suggest Rhodiola can slow down a liver enzyme called CYP2C9, which is one of the pathways your body uses to break down clomipramine. If that pathway gets blocked, clomipramine could build up a bit more than usual in your system. Since clomipramine isn't a prodrug (it doesn't need that enzyme to switch on), more drug hanging around means you might feel its effects, and its side effects, a little more strongly.

In real terms, that could mean extra drowsiness, dry mouth, constipation, blurry vision, dizziness, or a faster heartbeat. I want to be honest though: this is mostly theoretical and based on weak evidence, and the effect was small and varied between products. Still, it's worth a quick chat with your pharmacist or doctor before adding Rhodiola.

Here's the gist for Clomipramine (Anafranil). Your body uses a liver enzyme called CYP1A2 to help break this medicine down. In lab dishes, rhodiola can slow that enzyme a bit, which in theory could let clomipramine build up. If that happened, you might notice more of its usual side effects, like dry mouth, constipation, drowsiness, dizziness, or a faster heartbeat.

I want to keep this in perspective. This is mostly a theoretical concern based on test-tube work, the effect varies a lot between products, and a human study found rhodiola did not slow CYP1A2 in real life. So the risk looks low. Still, since clomipramine is a powerful drug, check with your pharmacist or doctor before adding rhodiola.

Here's the basic idea with Rhodiola and clomipramine (Anafranil). Clomipramine is partly broken down by a liver enzyme called CYP3A4. In the test tube, Rhodiola can slow that enzyme down, and if that happened in your body, clomipramine could build up a little. In theory that might mean more of the usual tricyclic side effects, like drowsiness, dry mouth, constipation, blurred vision, or a faster heartbeat.

That said, I wouldn't lose sleep over this one. It's rated minor, and a human study showed Rhodiola didn't actually affect a similar CYP3A4 drug, so the real-world effect looks weak and inconsistent. There's also a separate point worth raising: both can nudge serotonin, so check with your pharmacist or doctor before combining them.

A review of adverse event reports in Poland identified cases of tachyarrhythmias, myalgia, arthralgia, gum pain, restless leg syndrome, swallowing disorders, and changes in consciousness when rhodiola was taken in combination with paroxetine, escitalopram, fluoxetine, sertraline, trazodone, and/or duloxetine.

Here's the short version when it comes to pairing Rhodiola with Clomipramine: this is a theoretical concern, not a well-proven one. Clomipramine raises serotonin in the brain, and Rhodiola may gently nudge those same brain chemicals. In theory, stacking them could add up and bring on more side effects.

The evidence here is thin (mostly scattered case reports with other antidepressants, not Clomipramine itself), so I don't want to alarm you. Still, keep an eye out for things like a racing or irregular heartbeat, muscle or joint aches, restlessness, or feeling unusually foggy or agitated. If you notice anything like that, or before you add Rhodiola at all, please run it by your pharmacist or doctor first.
