P-GLYCOPROTEIN SUBSTRATES

Pomalidomide falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on pairing Rhodiola with your Pomalidomide (Pomalyst). Pomalidomide is carried around by a transporter pump called P-glycoprotein. In lab dish studies, Rhodiola seems to slow that pump down. The theory is that if the pump moves the drug less efficiently, a bit more pomalidomide could hang around in your body than expected.
If that happened, you might see more of pomalidomide's usual side effects, such as low blood counts (raising infection and bleeding risk), fatigue, or low platelets. I want to be honest though: this is only a theoretical concern from test-tube research, not real patients. Since pomalidomide is a serious cancer medicine, please don't add Rhodiola without first checking with your oncology team or pharmacist.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to keep in mind with pomalidomide (Pomalyst) and Rhodiola. Pomalidomide is taken to calm and shape the immune system as part of cancer treatment. Rhodiola, on the other hand, has shown in lab dishes and animal studies that it may stimulate the immune system. So in theory, the two could work against each other, and Rhodiola might blunt some of the effect your medication is meant to have.
I want to be honest that this concern comes only from early lab and animal research, not from studies in people, so we don't know if it actually plays out in real life. Still, because pomalidomide is a serious cancer therapy, I wouldn't add Rhodiola on your own. Please run it by your oncologist or pharmacist first so nothing interferes with your treatment.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body partly clears pomalidomide using a liver enzyme called CYP1A2. Rhodiola has been shown in lab (test tube) studies to slow that enzyme down. In theory, slowing it could let pomalidomide build up a bit, which might mean stronger effects and more of its usual side effects, like low blood counts, fatigue, infections, or a higher clot risk.
But I want to keep this in perspective. This is mostly a theoretical concern, the effect varies a lot between products, and a human study found rhodiola didn't actually block this enzyme for another CYP1A2 drug. Still, because pomalidomide is a serious cancer medicine, please don't add rhodiola without first checking with your oncologist or pharmacist.

In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Here's the gist for Pomalyst (pomalidomide). Your body clears this drug partly through a liver enzyme called CYP3A4. Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

What Is Pomalidomide Used For?
Brand name: Pomalyst
Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.
Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
