Why this applies to Pomalidomide

Pomalidomide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

If you're taking Pomalyst (pomalidomide) for multiple myeloma or Kaposi's sarcoma, here's what to know about turmeric. Pomalidomide is partly cleared by a liver enzyme called CYP3A4. Turmeric's main compound, curcumin, may slow that enzyme down. If it does, your body could clear pomalidomide more slowly and levels might creep up, which could mean more of its known side effects like low blood counts, fatigue, infections, or a higher clotting risk.

I want to be honest about the strength of this: it comes mostly from lab and animal studies, plus a handful of case reports with other cancer drugs, not solid human data with pomalidomide. The amounts in food are very unlikely to matter, but concentrated supplements are a different story. Since this is a serious cancer medicine, please run any turmeric supplement past your oncologist or pharmacist first.

Pomalidomide also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, turmeric might increase levels of drugs metabolized by CYP1A2. However, research is conflicting.
In vitro and animal research show that the turmeric constituent, curcumin, inhibits CYP1A2. However, other in vitro research suggests that curcumin does not significantly affect CYP1A2.

Here's the gist for Pomalidomide (Pomalyst): your body partly clears it using a liver enzyme called CYP1A2. In test tube and animal studies, the curcumin in turmeric may slow that enzyme down a little. If that happened in people, pomalidomide could clear a bit more slowly and build up, which in theory might mean stronger effects and more side effects like low blood counts, fatigue, or infections.

That said, this is theoretical and the lab research actually conflicts, with some studies showing no real effect. Everyday turmeric in cooking is very unlikely to matter. Concentrated curcumin supplements are the bigger question mark. Since pomalidomide is a serious cancer medicine, please run any turmeric supplement past your oncologist or pharmacist first.

Here's the idea behind this pairing. Pomalidomide (Pomalyst) is moved around your body partly by a transporter called P-glycoprotein, which acts like a gatekeeper. Lab and animal studies suggest the curcumin in turmeric may slow that gatekeeper down, which could let a little more pomalidomide get absorbed. In theory, that might nudge up its effects and side effects, things like low blood counts, fatigue, infection risk, or blood clots.

That said, this is only theoretical and based on test-tube and animal data, so it may not happen in real people. The amount of turmeric in cooking is very unlikely to matter. Concentrated curcumin capsules are the bigger question. Since pomalidomide is a serious cancer medicine, please check with your oncologist or pharmacist before adding any turmeric supplement.

About Pomalidomide

What Is Pomalidomide Used For?

Brand name: Pomalyst

Pomalidomide capsules are used in adults with multiple myeloma (a blood cancer affecting plasma cells) who have already tried at least two prior treatments — including lenalidomide and a proteasome inhibitor — and whose disease has continued to progress. It is given in combination with dexamethasone for this use.

Pomalidomide capsules are also used to treat Kaposi sarcoma (KS) — a cancer that can cause lesions on the skin and in internal organs — both in adults with AIDS-related KS after antiretroviral therapy has stopped working, and in adults with KS who are HIV-negative. The KS indication was approved based on response rate; ongoing studies are confirming long-term benefit.
