Here's the gist with fedratinib (Inrebic) and turmeric. This isn't about turmeric changing how fedratinib works in your blood. Instead, the worry is that both can be hard on the liver. Fedratinib already carries a known risk of liver enzyme changes, and turmeric (especially the concentrated, high-absorption supplement forms taken in big doses) has been linked in a handful of case reports to liver irritation too. Stacking two things that stress the liver is the concern here.

Honestly, this is theoretical and based on scattered anecdotes, not strong proof, so I don't want to alarm you. The turmeric you sprinkle on food is very unlikely to matter. But since your cancer team already monitors your liver labs, please loop in your pharmacist or doctor before adding any turmeric capsules.

Fedratinib Hydrochloride also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Your body clears fedratinib (Inrebic) largely through a liver enzyme called CYP3A4. Turmeric's curcumin can block that enzyme in lab studies, which would let fedratinib build up higher than intended. With this cancer medication, more drug in your system could mean worse side effects like nausea, diarrhea, vomiting, fatigue, or changes in liver and blood counts. There's a rare but serious warning about a brain condition (Wernicke's) too, so keeping levels steady matters.

Here's the honest part: this concern comes mostly from lab and animal studies, plus a few reports with similar cancer drugs, not solid human data on fedratinib itself. The curcumin in food seasoning is unlikely to cause trouble, but concentrated supplements are a different story. Please check with your oncology team or pharmacist before adding a turmeric supplement.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
