Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Turmeric interacts with.

Theoretically, turmeric might increase levels of drugs metabolized by CYP1A2. However, research is conflicting.
In vitro and animal research show that the turmeric constituent, curcumin, inhibits CYP1A2. However, other in vitro research suggests that curcumin does not significantly affect CYP1A2.

Here's the gist for Esbriet (pirfenidone): your liver clears it partly through an enzyme called CYP1A2. The curcumin in turmeric may, in theory, slow that enzyme down a little. If that happened, pirfenidone could build up slightly and you might notice more of its usual side effects, like nausea, upset stomach, tiredness, dizziness, or sun-sensitive skin. Pirfenidone is active on its own (not a prodrug), so blocking the enzyme tends to nudge its effect up, not down.

The honest part: this is theoretical, based only on lab and animal data, and even those studies disagree. Everyday turmeric in cooking is very unlikely to matter. Before starting a concentrated curcumin supplement, just run it by your pharmacist or doctor.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
