Here's the thing to know about pairing turmeric with methotrexate: both can be tough on the liver. Methotrexate is well known for raising liver enzymes and, over long-term use, can sometimes cause real liver injury. The worry is that adding turmeric (especially concentrated, high-dose supplements made to absorb better) might pile on a little extra stress to the liver. This isn't about turmeric changing how methotrexate works, it's simply two liver burdens stacking up.

I want to be honest that this concern is theoretical and based mostly on scattered case reports, not strong studies. The turmeric you sprinkle on food is very unlikely to be a problem. Before starting a turmeric supplement, please check with your pharmacist or doctor, since they already monitor your liver bloodwork on methotrexate.

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)

Methotrexate also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, turmeric might increase blood levels of OATP4C1 substrates.
In vitro research shows that the turmeric constituent curcumin competitively inhibits OATP4C1 transport. This transporter is expressed in the kidney and facilitates the renal excretion of certain drugs. Theoretically, taking turmeric might decrease renal excretion of OATP substrates.

Here's the idea behind pairing turmeric with methotrexate. Your kidneys clear methotrexate out of your body, and they rely on certain transporters to do that job. In test-tube studies, the curcumin in turmeric appears to block one of those transporters (OATP4C1). If that happened in real life, your kidneys might remove methotrexate a little more slowly, letting it build up.

Methotrexate has a narrow safety window, so even modest increases can matter, raising the risk of mouth sores, nausea, low blood counts, fatigue, or liver irritation. That said, this concern comes only from lab research, not from people, so it's theoretical. Cooking with turmeric as a spice is very unlikely to be an issue. Before starting a concentrated curcumin supplement, please check with your pharmacist or doctor first.

In one case report, a 39-year-old female taking methotrexate, turmeric, and linseed oil developed hepatotoxicity.

Here's the concern with pairing turmeric and methotrexate: both can be hard on your liver. The worry isn't that turmeric changes how much methotrexate is in your body, it's that the two together might add up to more strain on the liver than either alone. Methotrexate already carries a known risk of liver problems, which is why your doctor checks your liver enzymes with blood work.

Honestly, the evidence here is thin. It comes from a single case report of one woman who took methotrexate, turmeric, and linseed oil together, so this is more of a theoretical caution than a proven problem. Everyday spice amounts in cooking are very unlikely to matter. Concentrated curcumin supplements are where I'd be more careful. Before starting any turmeric supplement, please check with your pharmacist or doctor so they can keep an eye on your liver labs.

What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
