Kudzu + Methotrexate
Methotrexate brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex.
Methotrexate falls into 2 drug categories that Kudzu can interact with; the most serious is rated moderate. Theoretically, concomitant use might have additive hepatotoxic effects.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Kudzu interacts with whole categories of medication. Methotrexate falls into 2 of those categories, so it can interact with Kudzu in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
HEPATOTOXIC DRUGS
1 of 2Why this applies to Methotrexate
Methotrexate falls under HEPATOTOXIC DRUGS — the group Kudzu interacts with.
There is some concern that kudzu can adversely affect the liver.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about pairing kudzu with methotrexate: this isn't about kudzu changing how strong your methotrexate is. Instead, the worry is about your liver. Methotrexate is already known to be hard on the liver, especially when taken regularly for rheumatoid arthritis or psoriasis. There's some concern that kudzu may also stress the liver, so taking both together could, in theory, add up to more liver strain.
I want to be honest that this is a theoretical concern based on how each one behaves, not solid proof from studies. Still, since you already need regular liver blood tests on methotrexate, it's worth mentioning kudzu to your pharmacist or doctor before adding it. Don't change your methotrexate on your own.

METHOTREXATE (Trexall, others)
2 of 2Preclinical research suggests that kudzu extract greatly reduces the elimination and increases the toxicity of methotrexate. Kudzu might inhibit organic anion transporters (OATs) that are responsible for hepatobiliary and renal excretion of anions, similar to the interaction between methotrexate and non-steroidal anti-inflammatory drugs (NSAIDs).
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing kudzu and methotrexate. Your kidneys clear methotrexate out of your body using little transport proteins called organic anion transporters. Lab and animal studies hint that kudzu may block those transporters, which could slow how fast you get rid of the drug. If methotrexate hangs around longer than it should, levels can climb, and that raises the chance of side effects like mouth sores, nausea, low blood counts, fatigue, or liver and kidney strain.
I want to be honest about the evidence, though. This is theoretical and based only on preclinical research, not real patients. Still, methotrexate has a narrow safety window, so even small shifts matter. Please check with your doctor or pharmacist before adding kudzu.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Methotrexate page
“The liver is where methotrexate and supplements clash most. Methotrexate (Rheumatrex, Otrexup, Rasuvo), used for rheumatoid arthritis, psoriasis, and certain cancers, can strain the liver on its own, and 38 of the 82 interacting ingredients on this list add liver risk of their own, alcohol included, which makes this a list to approach…”

About Kudzu
What is Kudzu?
- Alcohol cravings and reduction
- Menopausal symptoms
- Heart and blood vessel health
- Hangover relief
- Traditional Chinese medicine (Ge Gen)
Kudzu is a fast-growing vine whose root has long been used in traditional Chinese medicine and is now studied mostly for reducing alcohol intake. Early research is promising for cutting back on drinking, but evidence for most other uses is weak. Talk with your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.
About Methotrexate
What Is Methotrexate Used For?
Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex
Methotrexate treats several kinds of cancer, plus rheumatoid arthritis, polyarticular juvenile idiopathic arthritis (pJIA, a form of arthritis in children that affects many joints), and severe psoriasis. Which uses apply depends on the specific product.
Methotrexate tablets, Trexall and Jylamvo (oral solution) are used for acute lymphoblastic leukemia (ALL), mycosis fungoides, relapsed or refractory non-Hodgkin lymphoma, rheumatoid arthritis, pJIA and severe psoriasis. Xatmep (oral solution) is only for ALL and pJIA in children. Methotrexate injection products cover a wider list of cancers. Otrexup and Rasuvo (subcutaneous auto-injectors) are for rheumatoid arthritis, pJIA and psoriasis only, and are not for cancer.

Methotrexate Is Linked to Lower Levels of One Nutrient
Natural Medicines records Methotrexate as a medicine that can lower these. This is not an interaction with Kudzu — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
