Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with kudzu and Tylenol (acetaminophen): both can put some stress on your liver. Acetaminophen is mostly cleared by the liver, and a small portion gets turned into a byproduct that can be hard on liver cells, especially at higher doses. There's also some concern that kudzu itself may affect the liver. So the worry is that pairing them could add up and tax your liver a bit more than either alone.

I want to be honest here: this is a theoretical concern based on how each is processed, not something proven in studies. For most people taking occasional, normal doses of Tylenol it's likely a minor issue. Still, stay within the labeled acetaminophen dose, skip alcohol, and check with your pharmacist or doctor before combining them, particularly if you have any liver problems.

From our pharmacist’s overview on the Acetaminophen page

“Acetaminophen (Tylenol) feels harmless because it sits on every shelf, but this list deserves real attention, mostly because of your liver. Of the 155 supplements and herbs on file, five carry a major rating, and the largest serious group, 38 ingredients, adds liver strain on top of acetaminophen's own well-known potential to harm the…”
