Methotrexate falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the simple version: both of these are tough on your liver, so stacking them can be a double hit. Methotrexate, even at the lower weekly doses used for psoriasis or rheumatoid arthritis, can raise liver enzymes and over time may cause liver scarring. Epistane is a methylated anabolic steroid that's also known to stress the liver. Put them together and the worry is that the liver strain adds up.

I want to be honest about the evidence: this concern is theoretical, based on how both compounds behave rather than reports of the exact pair causing harm. Methotrexate isn't a prodrug here, so this isn't about one speeding up or blocking the other. It's an additive liver risk. Watch for yellowing skin, dark urine, belly pain, or feeling unusually tired, and please talk with your doctor or pharmacist before combining them.

What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
