Nitrofurantoin falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the concern with pairing these two. Epistane is a methylated anabolic steroid that's hard on the liver, and nitrofurantoin (Macrobid, Macrodantin) can occasionally stress the liver too. On its own, liver trouble from a short course of nitrofurantoin is uncommon, but stacking it with something already taxing your liver could, in theory, add up to more strain.

This is a theoretical caution based on how these drugs behave, not on documented cases of the two together, so I don't want to alarm you. Still, watch for yellowing of your skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Honestly, my bigger worry is epistane itself, which is illegal to sell and genuinely liver-toxic. Please talk with your pharmacist or doctor before combining them.

About Nitrofurantoin

What Is Nitrofurantoin Used For?

Brand names: Macrodantin, Furadantin, Furatoin, Macrobid

Nitrofurantoin is used to treat urinary tract infections (UTIs) caused by certain susceptible bacteria. The capsule forms (including Macrodantin) are indicated for UTIs due to susceptible strains of Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for the same types of UTIs.

Nitrofurantoin is not used for kidney infections (pyelonephritis) or infections of the tissue around the kidney (perinephric abscesses). Because it works primarily in the urine and does not spread widely through body tissues, it may not fully clear the infection in some patients, and the UTI may return after treatment.
