Fingolimod falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the main concern with pairing these two: both can be tough on your liver. Fingolimod (Gilenya, Tascenso ODT) can raise liver enzymes in some people, which is why your doctor checks bloodwork before and during treatment. Epistane is a methylated anabolic steroid that's well known for stressing the liver. Put them together and you may stack two liver burdens on top of each other.

This warning is theoretical, based on how each one behaves rather than reported cases of the combo, so I don't want to alarm you. Still, signs like yellowing skin or eyes, dark urine, belly pain, or unusual fatigue would be worth acting on. Honestly, I'd steer clear of epistane entirely, and please loop in your doctor or pharmacist before mixing anything with fingolimod.

What Is Fingolimod Used For?

Brand names: Gilenya, Tascenso ODT

Fingolimod — available as capsules (Fingolimod, Fingolimod Hydrochloride, and Gilenya) and as an orally disintegrating tablet (Tascenso ODT) — is approved for the treatment of relapsing forms of multiple sclerosis (MS) in patients 10 years of age and older. This includes clinically isolated syndrome (a first MS episode), relapsing-remitting MS (the most common type with flare-ups and periods of stability), and active secondary progressive MS (a later stage where disability gradually worsens but relapses still occur).

All supported formulations share these same approved uses. The goal of treatment is to reduce the frequency of relapses and slow disease activity.
