Rifampin falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the simple version: both of these are tough on your liver. Rifampin, your TB medication, is well known for stressing the liver and can sometimes raise your liver enzymes or even cause more serious liver injury. Epistane is a methylated anabolic steroid that is also hard on the liver. Stack the two and the worry is that the liver strain could add up.

To be fair, this concern is theoretical, based on how these drugs behave rather than reports of people taking them together. Still, I'd take it seriously. Watch for yellowing of the skin or eyes, dark urine, belly pain, nausea, or unusual tiredness. Honestly, I'd steer clear of Epistane altogether (it's not a safe or legal supplement), and please loop in your doctor or pharmacist before combining anything with rifampin.

It is liver-toxic, can disrupt your natural hormones, and is illegal to sell as a supplement in the US under the Designer Anabolic Steroid Control Act. About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
