Methyltestosterone falls under HEPATOTOXIC DRUGS — the group Epistane interacts with.

Theoretically, concomitant use might have additive adverse hepatotoxic effects.
Epistane is an anabolic androgenic steroid, and more specifically a 17-alpha-alkylated androgen. Therefore, there is concern that epistane has hepatotoxic effects. Also, epistane is structurally similar to epitiostanol, which has been associated with hepatotixicity in a case series of four patients.

Here's the big thing to know: both Epistane and Methyltestosterone are 17-alpha-alkylated oral steroids. That chemical tweak is exactly what makes them hard on your liver. So stacking the two means you're doubling up on the same kind of liver stress, and the strain can add together.

What might that look like in real life? Things like yellowing of the skin or eyes, dark urine, belly pain, nausea, unusual tiredness, or itching. These are signs your liver may be struggling. Methyltestosterone on its own can also raise blood pressure and shift cholesterol, and Epistane piles onto that too. This concern is based on how these steroids work, not a head-to-head study, but the liver risk with methylated steroids is well recognized. Please don't combine them without talking to your doctor or pharmacist first.
