Why this applies to Methotrexate

Methotrexate falls under ORGANIC ANION TRANSPORTER 3 (OAT3) SUBSTRATES — the group Caffeic Acid interacts with.

Theoretically, caffeic acid might increase the levels and clinical effects of OAT3 substrates.
In vitro, caffeic acid inhibits OAT3. This drug transport protein is involved in renal tubular uptake of some drugs from the blood and then elimination in the urine. Inhibition of this transporters decreases renal elimination and increases drug levels in the body. Although caffeic acid inhibits OAT3 in vitro, the clinical significance of this in humans is not known.

Here's the gist with these two. Your kidneys rely on a transporter called OAT3 to help shuttle methotrexate out of your blood and into your urine. In lab dish studies, caffeic acid blocks that transporter. If the same thing happened in people, methotrexate could clear more slowly and build up to higher levels than expected.

Since methotrexate has a narrow safety window, higher levels could mean more of its known side effects, like mouth sores, nausea, low blood counts, fatigue, or liver and kidney strain. That said, this is purely theoretical right now, based only on test-tube data, and ordinary coffee or food amounts of caffeic acid likely won't matter. Before adding a concentrated caffeic acid supplement, please check with your pharmacist or doctor first.

What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
