Verapamil falls under HEPATOTOXIC DRUGS — the group Turmeric interacts with.
There is concern that turmeric might cause hepatotoxicity, especially when highly bioavailable formulations are used in high doses.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for turmeric and verapamil: this isn't about turmeric changing how verapamil works in your blood. Instead, the concern is that both can put a little extra stress on your liver. There have been a handful of reports tying concentrated turmeric (curcumin) products to liver irritation, especially the highly absorbable, high-dose formulas. Stack that on top of a medication your liver also has to process, and in theory the risk of liver trouble could add up.
I'll be honest, this is mostly theoretical and based on scattered anecdotes, not strong studies. The turmeric you sprinkle on food is very unlikely to matter. If you take a concentrated curcumin supplement, it's worth mentioning to your pharmacist or doctor, who can keep an eye on your liver if needed. Watch for things like yellowing skin or eyes, dark urine, or unusual tiredness, and call your provider if those show up.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the deal with these two. Your body clears verapamil using a liver enzyme called CYP3A4, and turmeric (mainly its curcumin) seems to nudge that enzyme in lab and animal studies. Most of that research points to turmeric slowing the enzyme, which could let verapamil build up a bit. If that happens, you might notice more of verapamil's usual effects: lower blood pressure, dizziness, a slower heartbeat, tiredness, or constipation. A few studies hint at the opposite (faster clearance, weaker effect), so the direction isn't fully settled.
The good news is the evidence is weak, mostly test tube and animal work, and the spice amounts you'd use in cooking are very unlikely to matter. Concentrated curcumin capsules are the bigger question mark. Before adding a turmeric supplement, just run it by your pharmacist or doctor.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro and animal research show that the turmeric constituent, curcumin, inhibits CYP1A2. However, other in vitro research suggests that curcumin does not significantly affect CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for you. The thinking is that curcumin (the active part of turmeric) might slow down a liver enzyme called CYP1A2, which can play a role in clearing some medicines. If your verapamil were broken down mainly by that enzyme, slowing it could let a bit more drug build up, possibly nudging effects like lower blood pressure, a slower heart rate, dizziness, or constipation.
That said, I want to be honest about how weak this is. It comes only from lab and animal studies, the results even disagree with each other, and verapamil is mostly handled by a different enzyme (CYP3A4), not CYP1A2. So normal cooking amounts of turmeric are very unlikely to matter. If you're considering concentrated curcumin capsules, just run it by your pharmacist or doctor first.

In vitro and animal research shows that curcuminoids and other constituents found in turmeric can inhibit P-glycoprotein expression and activity.

Here's the gist for verapamil: your body uses a little cellular pump called P-glycoprotein to push some of the drug back out of your gut and limit how much gets absorbed. Lab and animal studies hint that the curcumin in turmeric can slow that pump down. In theory, that could let a bit more verapamil into your bloodstream than usual.
If that happened, you might notice more of verapamil's typical effects, like lower blood pressure, dizziness, a slower heartbeat, or constipation. That said, this is very early evidence (test tubes and animals, not people), and the cooking amount of turmeric in your food is unlikely to do much. Concentrated curcumin capsules are a different story, so it's worth a quick chat with your pharmacist or doctor before starting one.

What Is Verapamil Used For?
Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil
Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.
Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
