Why this applies to Prednisolone

Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the short version on mixing turmeric with your prednisolone. Your body uses a liver enzyme called CYP3A4 to break this steroid down, and lab and animal studies suggest turmeric can nudge that enzyme in either direction. If it slows the enzyme, prednisolone may build up a bit, which could mean more of the usual steroid side effects like fluid retention, higher blood sugar, mood or sleep changes, and stomach upset. If it instead speeds the enzyme up, the steroid could clear faster and work less well.

The good news: this is based on test-tube and animal data, not solid human evidence, so a sprinkle of turmeric in cooking is very unlikely to matter. Concentrated curcumin capsules are where I'd pause. Just run it by your pharmacist or doctor before adding a supplement.

Theoretically, turmeric might increase the absorption of P-glycoprotein substrates.
In vitro and animal research shows that curcuminoids and other constituents found in turmeric can inhibit P-glycoprotein expression and activity.

Here's the gist with turmeric and prednisolone: turmeric's curcumin compounds may gently block a transporter pump called P-glycoprotein, which helps move certain drugs in and out of your cells. In theory, slowing that pump could let a bit more prednisolone get absorbed, nudging its levels and effects up slightly. With a steroid, that might mean a touch more of the usual things like trouble sleeping, mood changes, fluid retention, or a higher blood sugar.

That said, I want to be honest about the evidence here. This is rated minor and comes only from lab and animal studies, not real patients. Everyday cooking amounts of turmeric almost certainly won't matter. If you're taking concentrated curcumin supplements, just give your pharmacist or doctor a heads-up so they can keep an eye on things.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.
