Why this applies to Paclitaxel

Paclitaxel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Turmeric interacts with.

Turmeric might increase or decrease levels of drugs metabolized by CYP3A4.
In vitro and animal research show that turmeric and its constituents curcumin and curcuminoids inhibit CYP3A4. Also, 8 case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking turmeric and cancer medications that are CYP3A4 substrates, including everolimus, ruxolitinib, ibrutinib, and palbociclib, and bortezomib. In another case report, a transplant patient presented with acute nephrotoxicity and elevated tacrolimus levels after consuming turmeric powder at a dose of 15 or more spoonfuls daily for ten days prior. It was thought that turmeric increased levels of tacrolimus due to CYP3A4 inhibition.
Conversely, other in vitro research suggests that turmeric induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. An animal model suggests that induction of CYP3A4 occurs after daily curcumin use for 1 week. However, the induction of CYP3A4 by turmeric has not been reported in humans.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help clear paclitaxel out of your system. Lab and animal studies suggest the curcumin in turmeric can block that enzyme. If that happens, paclitaxel could hang around at higher levels than expected, which may mean stronger chemo side effects like low blood counts, infections, nerve tingling or numbness in the hands and feet, fatigue, and muscle or joint aches.

I want to be honest about the evidence, though. This is mostly test tube and animal data, not solid human studies, and some research even hints turmeric could push the enzyme the other way. Cooking with turmeric as a spice is very unlikely to matter. Concentrated curcumin capsules are the real question. Please run any turmeric supplement past your oncology team before using it.

Paclitaxel also falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP).

Theoretically, turmeric might increase blood levels of OATP4C1 substrates.
In vitro research shows that the turmeric constituent curcumin competitively inhibits OATP4C1 transport. This transporter is expressed in the kidney and facilitates the renal excretion of certain drugs. Theoretically, taking turmeric might decrease renal excretion of OATP substrates.

Here's the gist: paclitaxel is a chemotherapy drug your body normally clears in part through transporter proteins. The concern with turmeric is that one of its compounds, curcumin, can block a kidney transporter called OATP4C1 in lab tests. In theory, slowing that exit route could let a little more paclitaxel linger in your system, which might mean stronger side effects like low blood counts, numbness or tingling in the hands and feet, fatigue, or hair loss.

But I want to be honest about the strength of this: right now it's only based on test-tube and animal research, not real patients, so we don't truly know if it matters in people. Since you're dealing with cancer treatment, please don't add a turmeric supplement without first checking with your oncologist or pharmacist. Food-amount turmeric in cooking is far less of a worry than concentrated capsules.

Here's the gist for paclitaxel (Taxol, Abraxane). Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs out of cells. Turmeric's curcumin may slow that pump down, at least in lab and animal studies. In theory, that could let a bit more paclitaxel build up in your system, which might mean stronger side effects like low blood counts, nerve tingling in the hands and feet (neuropathy), or worse fatigue.

The good news: this is only a theoretical, minor concern based on test-tube data, not human studies, and food amounts of turmeric in cooking are very unlikely to matter. Still, paclitaxel is serious chemotherapy, so please clear any concentrated turmeric supplement with your oncology team before using it.

Theoretically, turmeric might alter blood levels of paclitaxel, although any effect may not be clinically relevant.
Clinical research in adults with breast cancer receiving intravenous paclitaxel suggests that taking turmeric may modestly alter paclitaxel pharmacokinetics. Patients received paclitaxel on day 1, followed by either no treatment or turmeric 2 grams daily from days 2-22. Pharmacokinetic modeling suggests that turmeric reduces the maximum concentration and area under the curve of paclitaxel by 12.1% and 7.7%, respectively. However, these changes are not likely to be considered clinically relevant. Conversely, animal research suggests that curcumin, a constituent of turmeric, enhances the oral bioavailability of paclitaxel. However, the significance of this interaction is unclear, as this drug is typically administered intravenously in clinical settings.

If you are getting paclitaxel infusions for cancer and wondering about your turmeric capsules, here is the honest picture. The concern on file is mostly theoretical. In one study of women with breast cancer, taking 2 grams of turmeric daily slightly lowered paclitaxel blood levels, about 8 to 12 percent. That is a small dip, and researchers did not consider it large enough to change how well the drug works or how you feel.

The evidence here is weak, coming mostly from lab and animal work, so I would not lose sleep over it. Still, because paclitaxel is serious chemotherapy, please let your oncology team know about any turmeric supplement you take. They are the right people to decide if it fits with your treatment plan.

What Is Paclitaxel (with albumin) Injection Used For?

Brand names: Taxol, Abraxane

Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound) is used to treat three types of cancer. It treats metastatic breast cancer in patients whose prior chemotherapy has stopped working or who relapsed within 6 months of completing adjuvant (post-surgery) chemotherapy. It is also used as a first-line treatment for locally advanced or metastatic non-small cell lung cancer (NSCLC) — given together with carboplatin — in patients who cannot have surgery or radiation therapy.

Additionally, it is approved as a first-line treatment for metastatic adenocarcinoma of the pancreas, given in combination with gemcitabine. All three uses apply specifically to Paclitaxel Protein-Bound Particles for Injectable Suspension (Albumin-Bound), including the brands Abraxane and related albumin-bound formulations.
