Methotrexate falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Sweet Orange interacts with.

Consuming sweet orange juice can decrease oral absorption of OATP substrates. Separate administration by at least 4 hours.
Clinical research shows that consuming sweet orange juice inhibits OATP, which reduces bioavailability of oral drugs that are substrates of OATP. For example, sweet orange juice decreases bioavailability of fexofenadine, a substrate of OATP, by about 72% and of celiprolol, another OATP substrate, by up to 90%. Since sweet orange juice seems to affect OATP for a short time, recommend separating drug administration and consumption of sweet orange juice by at least 4 hours.

Here's the gist for methotrexate taken by mouth: your gut uses a transporter called OATP to help pull the drug into your bloodstream. Sweet orange juice can block that transporter for a few hours, which may mean less methotrexate actually gets absorbed. For a drug you rely on to calm rheumatoid arthritis or psoriasis, that could translate into weaker control of your symptoms, like more joint pain, stiffness, or flare-ups, rather than more side effects.

The good news is this effect is short-lived. An easy fix is timing: keep your orange juice and your methotrexate dose at least 4 hours apart. Before changing anything, please run it by your pharmacist or doctor, especially since methotrexate dosing is precise.

What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
