P-GLYCOPROTEIN SUBSTRATES
Why this applies to Minocycline
Minocycline falls under P-GLYCOPROTEIN SUBSTRATES — the group Sweet Orange interacts with.
Animal and in vitro research suggest that orange juice extract inhibits drug efflux by P-gp, increasing absorption and levels of P-gp substrates. In contrast, pharmacokinetic research in humans shows that drinking large amounts of sweet orange juice decreases absorption and levels of the P-gp substrate celiprolol. This suggests that orange juice actually induces drug efflux by P-gp or affects drug levels by another mechanism such as inhibiting the gut drug transporter called organic anion transporting polypeptide (OATP). Until more is known, sweet orange juice should be used cautiously in people taking P-gp substrates.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for minocycline and sweet orange. Minocycline is a ready-to-go antibiotic (not a prodrug), and it can be moved around by a gut transporter called P-glycoprotein. Orange juice seems to nudge this transporter, but the human research points in a confusing direction. In one study, drinking large amounts of orange juice actually lowered blood levels of a similar drug, which could mean less antibiotic gets absorbed and a weaker punch against your infection.
The evidence here is fairly weak and based on a different medicine, so I wouldn't lose sleep over a normal glass with breakfast. Still, if you're treating a real infection, it's smart to take minocycline with water and run any big orange juice habit by your pharmacist or doctor.

What Is Minocycline Used For?

Oral and intravenous minocycline products treat many serious bacterial infections, including Rocky Mountain spotted fever, respiratory tract infections, sexually transmitted infections such as lymphogranuloma venereum and nongonococcal urethritis, plague, cholera, brucellosis, tularemia, and others caused by susceptible organisms. MINOCIN Intravenous is used when oral therapy is not adequate or tolerated.
Extended-release oral minocycline tablets are indicated specifically for inflammatory lesions of moderate to severe acne vulgaris in patients 12 years and older — not for treating infections. AMZEEQ (topical foam) treats inflammatory lesions of moderate to severe acne vulgaris in patients 9 years and older. ZILXI (topical foam) and EMROSI (oral capsule) are indicated for inflammatory lesions of rosacea in adults. ARESTIN (oral powder placed by a dental professional) is used alongside scaling and root planing to reduce pocket depth in adult periodontitis.
