Methotrexate falls under HEPATOTOXIC DRUGS — the group Vitamin A interacts with.

Theoretically, taking high doses of vitamin A in combination with other potentially hepatotoxic drugs might increase the risk of liver disease.
The tolerable upper intake level (UL) is the highest level of intake that is likely to pose no risk of adverse effects. Doses of vitamin A above the UL can cause hepatotoxicity, ranging from elevated liver enzymes to liver failure.

Here's the thing to know about pairing vitamin A with methotrexate: both can be tough on the liver. Methotrexate is well known for raising liver enzymes and, over time, causing liver scarring, which is why your doctor checks your bloodwork regularly. The concern is that stacking high-dose vitamin A on top could add to that liver stress.

This is a theoretical caution based on expert opinion, not strong studies, so try not to panic. Normal food sources and a standard multivitamin are very unlikely to be a problem. The worry is concentrated, high-dose vitamin A supplements taken above the recommended upper limit. Before adding any vitamin A pills, check with your pharmacist or doctor so they can keep an eye on your liver tests.

What Is Methotrexate Used For?

Brand names: Rheumatrex, Otrexup, Rasuvo, Reditrex

Methotrexate treats a wide range of serious conditions. In cancer care, it is used for acute lymphoblastic leukemia, non-Hodgkin lymphoma, osteosarcoma, breast cancer, head and neck cancer, and gestational trophoblastic neoplasia (a rare tumor related to pregnancy). It is also used to prevent and treat meningeal leukemia (cancer affecting the lining of the brain and spinal cord).

Outside of cancer, methotrexate is prescribed for adults with rheumatoid arthritis (RA), children with polyarticular juvenile idiopathic arthritis (a form of childhood arthritis), and adults with severe psoriasis that hasn't responded well to other treatments.
