Turmeric and Estradiol, Norgestimate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Turmeric interacts with whole categories of medication. Estradiol, Norgestimate falls into 3 of those categories, so it can interact with Turmeric in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

1 of 3

Why this applies to Estradiol, Norgestimate

Estradiol, Norgestimate falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Turmeric interacts with.

Theoretically, turmeric might increase levels of drugs metabolized by CYP1A2. However, research is conflicting.
In vitro and animal research show that the turmeric constituent, curcumin, inhibits CYP1A2. However, other in vitro research suggests that curcumin does not significantly affect CYP1A2.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Ortho-Prefest (estradiol and norgestimate): both are active hormones, not prodrugs, so we don't have to worry about turmeric switching them on. The concern on file is that curcumin, the active part of turmeric, might slow down a liver enzyme (CYP1A2) that helps clear estrogen. In theory, that could let a little more estradiol linger, which can mean things like breast tenderness, nausea, bloating, or breakthrough spotting.

Honestly though, this is rated minor and rests only on lab and animal data, and even those studies disagree. Cooking with turmeric in food almost certainly won't matter. If you take concentrated curcumin capsules daily, just mention it to your pharmacist or doctor so they can keep an eye on things.

ESTROGENS

2 of 3

Estradiol, Norgestimate also falls under ESTROGENS.

Theoretically, large amounts of turmeric might interfere with hormone replacement therapy through competition for estrogen receptors.
In vitro research shows that curcumin, a constituent of turmeric, displaces the binding of estrogen to its receptors.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Ortho-Prefest (estradiol and norgestimate): this is a hormone replacement combo, and turmeric's main compound, curcumin, has been shown in lab dish studies only to nudge estrogen off its receptors. The thinking is that very high amounts of curcumin might compete with your estrogen a little, which could in theory soften how well the hormones work (think a return of symptoms like hot flashes or night sweats).

That said, this is rated minor and rests on test-tube data, not real people. Everyday cooking with turmeric is very unlikely to matter. Concentrated curcumin capsules are where I'd pause, so just run those by your pharmacist or doctor before starting them alongside your HRT.

P-GLYCOPROTEIN SUBSTRATES

3 of 3

Estradiol, Norgestimate also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, turmeric might increase the absorption of P-glycoprotein substrates.
In vitro and animal research shows that curcuminoids and other constituents found in turmeric can inhibit P-glycoprotein expression and activity.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with your Prefest (estradiol plus norgestimate). Turmeric, in lab and animal studies, can block a transport pump called P-glycoprotein that helps move certain hormones in and out of cells. The thinking is that turmeric might let a little more of your hormone medicine get absorbed. In theory, that could mean slightly stronger effects, like breast tenderness, nausea, bloating, or breakthrough spotting.

That said, this is rated minor and rests only on test-tube and animal data, not real people. The amount of turmeric you'd get from cooking is very unlikely to matter. If you're taking concentrated curcumin capsules and notice new symptoms, just check in with your pharmacist or doctor before changing anything.

About Turmeric

What is Turmeric?

Turmeric
  • Joint pain and arthritis
  • Inflammation
  • Digestive upset
  • Antioxidant support
  • Skin conditions

Turmeric is a popular spice whose main active compounds, curcuminoids, are studied mostly for inflammation and joint pain. Some research is promising, but quality is mixed and curcumin is poorly absorbed on its own. For most people, dietary amounts are safe, but talk to your pharmacist or doctor before taking concentrated supplements, especially if you are pregnant, nursing, or have a medical condition.
