Why this applies to Bexarotene
Bexarotene falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body clears bexarotene largely through a liver enzyme called CYP3A4. Rhodiola has shown in test-tube studies that it can slow that same enzyme down. In theory, that could let bexarotene build up a bit, which might mean more of its usual side effects, things like high triglycerides and cholesterol, an underactive thyroid, headaches, or low white blood cell counts.
The honest truth, though, is that this concern is mostly theoretical and the evidence is weak. The lab effect was inconsistent, and a human study found rhodiola did not meaningfully slow CYP3A4. Still, because bexarotene is a serious cancer medicine that's closely monitored, please run any rhodiola use past your oncologist or pharmacist first.

Bexarotene capsules (brands: Bexarotene, Targretin) are used to treat the skin symptoms of cutaneous T-cell lymphoma (CTCL) in patients whose disease has not responded to at least one prior systemic treatment.
Bexarotene gel 1% (brands: Bexarotene, Targretin) is used to treat the skin lesions of early-stage CTCL (Stage IA and IB) when other therapies have not worked or could not be tolerated. The gel is a topical therapy and is not intended for systemic use.
