Rhodiola and Citalopram

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Rhodiola interacts with whole categories of medication. Citalopram falls into 2 of those categories, so it can interact with Rhodiola in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

1 of 2

Why this applies to Citalopram

Citalopram falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Severity Minor Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Here's the gist for citalopram and rhodiola. In lab dishes, rhodiola can slow down an enzyme called CYP3A4, which is one of the pathways your body uses to clear citalopram. In theory, that could let citalopram build up a little, which might mean more of the usual side effects like drowsiness, nausea, dry mouth, or feeling jittery. But this is mostly test-tube data, and a human study found rhodiola did not meaningfully slow this enzyme in real life, so the actual risk looks low.

One thing I'd keep an eye on separately is that both rhodiola and citalopram can nudge serotonin upward, so I'd mention this combo to your pharmacist or doctor before starting it. Don't change your citalopram on your own.

ANTIDEPRESSANT DRUGS

2 of 2

Citalopram also falls under ANTIDEPRESSANT DRUGS.

Theoretically, rhodiola might increase the risk of adverse effects when taken with antidepressants.
A review of adverse event reports in Poland identified cases of tachyarrhythmias, myalgia, arthralgia, gum pain, restless leg syndrome, swallowing disorders, and changes in consciousness when rhodiola was taken in combination with paroxetine, escitalopram, fluoxetine, sertraline, trazodone, and/or duloxetine.
Severity Minor Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

If you're taking citalopram (Celexa) for depression, here's the simple version: rhodiola seems to nudge some of the same brain chemicals, like serotonin, that your medication already raises. Stacking the two could, in theory, add up to more serotonin activity than you want. Citalopram by itself can cause side effects like nausea, headache, drowsiness, restlessness, or a faster heartbeat, and adding rhodiola might make those a little more likely.

I want to be honest about the evidence, though. This is mostly theoretical and based on scattered case reports, not strong studies, and it's rated minor. That doesn't mean ignore it. Before adding rhodiola, please run it by your pharmacist or doctor so they can check it against your full medication list.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

About Rhodiola

What is Rhodiola?

Rhodiola
  • Fatigue and low energy
  • Stress and coping (adaptogen)
  • Mental performance and focus
  • Mild depression
  • Physical and exercise performance

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. It is generally well tolerated for short-term use, but talk to your pharmacist or doctor before trying it, and avoid it during pregnancy and breastfeeding.

About Citalopram

What Is Citalopram Used For?

Brand name: Celexa

Citalopram — available as tablets (including brand Celexa), oral solution, and capsules — is used to treat major depressive disorder (MDD) in adults. This includes depression that causes persistent low mood, loss of interest in everyday activities, changes in sleep or appetite, fatigue, and difficulty concentrating.

Citalopram capsules are specifically indicated for adults with MDD but are not intended for starting or stopping treatment — another citalopram product must be used for dose titration. All forms are indicated for adults only; citalopram tablets are not approved for use in children.
