Acalabrutinib falls under P-GLYCOPROTEIN SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of P-glycoprotein substrates.
In vitro research shows that rhodiola inhibits P-glycoprotein. Theoretically, using rhodiola with P-glycoprotein substrates might increase drug levels and potentially increase the risk of adverse effects.

Here's the gist for Calquence (acalabrutinib). Your body uses a kind of cellular bouncer called P-glycoprotein to help move this drug along. Lab studies (in test tubes, not people) suggest rhodiola may slow that bouncer down. In theory, that could let more acalabrutinib build up in your system, which might mean a bit more of its usual side effects, like headaches, bruising or bleeding more easily, infections, or feeling unusually tired.

I want to be straight with you: this is theoretical and based only on early lab work, so we don't know if it truly matters in real life. Still, acalabrutinib is a serious cancer medicine, so don't add rhodiola without first checking with your oncologist or pharmacist.

Acalabrutinib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Your body relies on a liver enzyme called CYP3A4 to clear acalabrutinib (Calquence) out of your system. In test-tube studies, rhodiola has shown some ability to slow that enzyme down. If that happened in real life, acalabrutinib could build up a bit more than expected, which in theory might mean more of its known side effects, things like headaches, bruising or bleeding, fatigue, or a higher risk of infection.

Here's the reassuring part: this concern is mostly theoretical. The effect varies a lot by product, and a human study found rhodiola did not actually change how a similar CYP3A4 drug was processed. Still, because acalabrutinib is a serious cancer medicine, please run any rhodiola use past your oncologist or pharmacist first.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Some small studies suggest it may modestly reduce fatigue and improve mood, but the evidence is limited and not strong. What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
