Theoretically, taking rhodiola with antihypertensive drugs might increase the risk of hypotension.
In vitro and animal research shows that rhodiola extract inhibits angiotensin-converting enzyme (ACE) and might lower blood pressure.

Here's the simple version: Aliskiren already works to lower your blood pressure. Rhodiola, at least in lab dishes and animal studies, seems to have a mild blood-pressure-lowering action of its own. So when you stack the two together, the worry is that your pressure could drop a bit more than expected. That might leave you feeling dizzy, lightheaded, tired, or a little woozy when you stand up too fast.

I want to be honest about the strength of this, though. It's mostly theoretical and based on early research, not on real patient cases, so for many people it won't cause noticeable trouble. Still, if you take Aliskiren and want to try Rhodiola, check in with your pharmacist or doctor first, and keep an eye on your blood pressure when starting.

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Here's the short version: your blood pressure pill aliskiren is partly handled by a liver enzyme called CYP3A4. In a test tube, rhodiola can slow that enzyme down a bit. If that happened in your body, aliskiren might build up slightly, which could nudge its effects a little stronger (think lightheadedness, dizziness, or blood pressure dropping more than expected). Aliskiren is not a prodrug, so it doesn't get weaker from this.

The honest truth, though, is that this is mostly theoretical. A real human study found rhodiola did not actually slow CYP3A4 in the way the lab work suggested, and the effect varies a lot by product. I'd rate the risk as small. Still, if you take aliskiren, just mention rhodiola to your pharmacist or doctor before starting it.

Rhodiola is an herb traditionally used to fight fatigue and help the body cope with stress. Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.

Managing blood pressure is usually part of a broader plan that may also include diet changes, exercise, cholesterol control, and other medications. Many people need more than one blood pressure drug to reach their goal.
