Amitriptyline, Perphenazine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). The concern here is that rhodiola may mildly block a liver enzyme called CYP2C9, which helps clear amitriptyline. If that enzyme slows down, more of the drug can hang around in your body, which could nudge up its usual side effects, things like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or a faster heartbeat.
I'll be honest though: this is mostly theoretical. The lab data is inconsistent, and one small human study showed only a modest 21% change with a different drug. Amitriptyline actually leans more on other enzymes too. Don't start or stop anything on your own, just give your pharmacist or doctor a heads-up before adding rhodiola.

This combo pill pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). The amitriptyline part is partly cleared by a liver enzyme called CYP1A2. In the lab, rhodiola has shown some ability to slow that enzyme down, which in theory could let amitriptyline build up a little and make its usual side effects more noticeable, things like drowsiness, dry mouth, constipation, blurry vision, or feeling lightheaded when you stand.
That said, the real-world evidence here is weak. A human study found rhodiola did not actually slow down caffeine, another CYP1A2 drug, and this effect varies a lot by product. I would not panic, but do mention rhodiola to your pharmacist or doctor before starting it so they can keep an eye on how you feel.

In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo (the older Triavil/Etrafon pills that pair amitriptyline with perphenazine). Your body uses an enzyme called CYP3A4 to help break both of these down. In a test tube, rhodiola can slow that enzyme a bit, and in theory that could let these medicines build up slightly, which might mean more of their usual side effects like drowsiness, dry mouth, constipation, blurred vision, or feeling lightheaded when you stand.
That said, this is a minor, mostly theoretical concern. The lab effect was inconsistent, and a human study found rhodiola didn't actually slow CYP3A4. So I wouldn't lose sleep over it, but since these are sedating drugs, it's worth a quick word with your pharmacist or doctor before adding rhodiola.

CNS DEPRESSANTS

A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.

Your combo pill, Triavil, pairs amitriptyline (an older antidepressant) with perphenazine (an antipsychotic). Both already have a calming, sedating side to them. The concern here is simply additive: Rhodiola could add a little extra to that effect, so theoretically you might feel sleepier or more lightheaded than usual. Things like next-day grogginess, dizziness when you stand up (a drop in blood pressure), or even some muscle twitching have been reported when rhodiola was paired with other sedating medicines.
To be fair, this is theoretical and based on a handful of anecdotal reports, not strong studies, so I wouldn't lose sleep over it. Still, since both of your medicines affect the brain, it's worth a quick chat with your pharmacist or doctor before adding rhodiola.

ANTIDEPRESSANT DRUGS

A review of adverse event reports in Poland identified cases of tachyarrhythmias, myalgia, arthralgia, gum pain, restless leg syndrome, swallowing disorders, and changes in consciousness when rhodiola was taken in combination with paroxetine, escitalopram, fluoxetine, sertraline, trazodone, and/or duloxetine.

This combination product pairs amitriptyline (a tricyclic antidepressant) with perphenazine (an older antipsychotic). The concern here is pretty soft. We don't have solid studies showing rhodiola reliably reacts with this exact medication. What we do have are scattered, anecdotal reports where people taking rhodiola alongside various antidepressants noticed things like a fast or irregular heartbeat, muscle aches, restless legs, or feeling off mentally.
Since amitriptyline already carries its own risk of a racing heart, dry mouth, drowsiness, and dizziness, the worry is that rhodiola could nudge those in the same direction. It's a theoretical, minor concern rather than a proven danger, but I'd still loop in your pharmacist or doctor before adding rhodiola so they can keep an eye on you.
