Why this applies to Sulfamethoxazole, Trimethoprim
Sulfamethoxazole, Trimethoprim falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Rhodiola interacts with.
In vitro research shows that rhodiola inhibits CYP2C9. This effect is highly variable and appears to be dependent on the rhodiola product studied. Also, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days reduces the metabolism of losartan, a CYP2C9 substrate, by 21% after 4 hours.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
If you're taking Bactrim (sulfamethoxazole and trimethoprim) and thinking about adding rhodiola, here's the gist. The sulfamethoxazole part is partly broken down by a liver enzyme called CYP2C9. In lab and small human studies, rhodiola can mildly slow that enzyme down. In theory, that could let a bit more of the antibiotic linger in your body, which might nudge up the chance of side effects like nausea, upset stomach, rash, or sun sensitivity.
Honestly, this is on the weaker, mostly theoretical end of the evidence, and the effect seen with other drugs was small. For a short antibiotic course it's unlikely to be a big deal, but it's worth a quick chat with your pharmacist or doctor before combining them, especially if you notice any new symptoms.
