Belladonna, Ergotamine, Phenobarbital falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that rhodiola inhibits CYP3A4. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of midazolam, a CYP3A4 substrate.

Here's the gist with Bellergal-S, which combines belladonna, ergotamine, and phenobarbital. In lab dishes, rhodiola can block the liver enzyme CYP3A4, and your body uses that same enzyme to help clear the ergotamine part of this medicine. In theory, slowing that breakdown could let ergotamine build up a bit, which might mean more side effects like nausea, tingling or coldness in the fingers and toes, leg cramps, or a faster or slower heartbeat.

The honest news is that this is mostly theory. The effect varies a lot by product, and a human study found rhodiola did not actually slow a similar CYP3A4 drug. So real risk is probably small, but ergotamine doesn't leave much room for error. Please run it by your pharmacist or doctor before combining them.

Theoretically, rhodiola might increase the risk of adverse effects when taken with CNS depressants.
A review of adverse event reports in Poland identified cases of excessive sedation, myoclonus, hypotension, and hallucinations when rhodiola was taken with haloperidol, diazepam, or alprazolam.

The piece of Bellergal-S that matters here is the phenobarbital, a barbiturate that calms the brain. Rhodiola is sometimes thought to nudge the nervous system, and there are a few scattered reports of people feeling overly sedated or even seeing things when they combined rhodiola with other sedating medicines. So in theory, pairing it with phenobarbital could add to that drowsy, slowed-down feeling, leaving you more sleepy, foggy, dizzy, or unsteady than usual.

I want to be honest: this is weak, anecdotal evidence, and the risk is rated minor. For most people it may not cause trouble at all. Still, since phenobarbital is already a strong sedative, it is worth a quick chat with your pharmacist or doctor before adding rhodiola.
