Here is the interaction we found between these two, with the evidence behind it.
CNS DEPRESSANTS
Why this applies to Belladonna, Ergotamine, Phenobarbital
Belladonna, Ergotamine, Phenobarbital falls under CNS DEPRESSANTS — the group 1,4-butanediol interacts with.
1,4-Butanediol is metabolized to gamma-hydroxybutyrate (GHB). Concomitant use of CNS depressants with GHB may cause serious CNS and respiratory depression.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
First things first: 1,4-butanediol isn't a real dietary supplement. Once swallowed, your body turns it into GHB, a strong brain depressant. That matters here because the medication you asked about, Bellergal-S, contains phenobarbital, a barbiturate that already slows down the brain and breathing all on its own.
Put the two together and you're stacking one sedative on top of another. The realistic worry is dangerous over-sedation: heavy drowsiness, confusion, very slow or shallow breathing, and in a worst-case situation passing out or stopping breathing. This is a major concern even though the evidence is mostly anecdotal. Please don't use 1,4-butanediol, and talk with your doctor or pharmacist about anything you're taking alongside Bellergal-S.

From our pharmacist’s overview on the Belladonna, Ergotamine, Phenobarbital page
“Bellergal-S packs three drugs into one tablet, and its supplement list earns real respect before you mix in anything else. Phenobarbital is a sedating barbiturate, belladonna adds drying anticholinergic effects, and ergotamine constricts blood vessels, so this combination collects interactions from several directions at once.”
