Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's a wrinkle worth knowing about Thiotepa. It's actually a prodrug, meaning your body has to switch it on. The enzyme CYP3A4 helps convert Thiotepa into its active cancer-fighting form (TEPA). Now, the plumbagin in Ceylon Leadwort has been shown in lab dishes to block CYP3A4. In theory, slowing that enzyme could change how much active drug you actually form, which matters a lot with a chemotherapy this powerful.

I want to be honest about the evidence though: this is only test-tube and animal data, never confirmed in real people, so the practical risk is uncertain. Still, with a chemo drug, I wouldn't gamble. Please don't add or stop anything on your own. Run any supplement past your oncology team or pharmacist first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2B6 (CYP2B6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2B6. Drugs that are metabolized by CYP2B6 include ketamine (Ketalar), phenobarbital, orphenadrine (Norflex), secobarbital (Seconal), and dexamethasone (Decadron).

Here's the wrinkle with thiotepa: it's actually a prodrug. Your body has to convert it into its active cancer-fighting form, and the enzyme CYP2B6 is one of the main players that does that activation. Ceylon Leadwort contains plumbagin, which in lab studies appears to block (inhibit) CYP2B6. So in theory, if that enzyme is slowed down, your body might make less of thiotepa's active form, which could mean less of the cancer-fighting effect you're counting on.

I want to be honest about the evidence though. This is based only on test-tube work, not real people, so it's theoretical. Since thiotepa is a serious chemotherapy drug, please don't add this supplement without first running it by your oncology team or pharmacist.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
