Prednisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for prednisone: your liver uses an enzyme called CYP3A4 to help clear it from your body. In test-tube studies, plumbagin (the main active compound in Ceylon Leadwort) appears to slow that enzyme down. If that happened in real life, prednisone could hang around a bit longer and its effects could feel stronger. With a steroid, that can mean more of the usual side effects, things like trouble sleeping, mood swings, a bigger appetite, fluid retention, higher blood sugar, or stomach upset.

I want to be honest, though: this is only laboratory evidence and hasn't actually been seen in people, so the real risk is uncertain. Still, it's worth a quick chat with your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the thing to know if you're taking prednisone for something like arthritis, lupus, asthma flares, or a serious allergic reaction. Prednisone works by calming down your immune system. Lab and animal studies suggest that plumbagin, a compound in Ceylon Leadwort, also has its own immune-affecting activity. The worry is that the two could pull in different directions and make your prednisone less predictable, possibly weakening the control of whatever it's treating.

I want to be honest about the evidence here: this is theoretical, based only on lab and animal data, not human studies, so I wouldn't lose sleep over it. Still, since prednisone is often treating something important, it's worth a quick chat with your pharmacist or doctor before adding this supplement rather than starting it on your own.
