Fentanyl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2D6 (CYP2D6) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2D6. Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), codeine, desipramine (Norpramin), flecainide (Tambocor), fluoxetine (Prozac), ondansetron (Zofran), tramadol (Ultram), and others.

Here's the thing to know about pairing Ceylon Leadwort with Fentanyl. The plant's main compound, plumbagin, has been shown in test-tube studies to block an enzyme called CYP2D6. In theory, blocking that enzyme could slow how your body clears certain medications, letting them build up.

I want to be honest about the evidence here though. It's very weak, coming from lab dishes, not real people, and CYP2D6 is actually a minor player in how fentanyl is handled (the liver mostly uses a different enzyme, CYP3A4). So a meaningful jump in fentanyl levels from this supplement isn't well supported. Still, because fentanyl is so potent, even small increases could mean extra drowsiness, slowed breathing, or confusion. Please check with your pharmacist or doctor before combining them.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for fentanyl. Your body relies on a liver enzyme called CYP3A4 to break fentanyl down and clear it out. Plumbagin, a compound in Ceylon Leadwort, has been shown in lab dish studies to slow that same enzyme down. If that were to happen in real life, fentanyl could build up in your system and hit harder than expected.

With a potent opioid like fentanyl, that's worth taking seriously. Too much can cause heavy sedation, confusion, and dangerously slow or shallow breathing. I want to be honest, though: this interaction has only been seen in test tubes, never confirmed in people, so we don't know how strong it really is. Please don't change anything on your own. Loop in your doctor or pharmacist before combining these.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Fentanyl Used For?

Brand names: Sublimaze, Duragesic, Fentora, Lazanda, Abstral, Subsys

Fentanyl transdermal system is prescribed for severe, persistent pain that cannot be controlled with other pain medicines, including shorter-acting opioids. It is only for people who are already opioid-tolerant — meaning they've been taking a significant daily opioid dose for at least a week.

It is not for short-term pain, post-surgical pain, mild pain, or use on an as-needed basis. Because of serious risks including addiction, overdose, and death, it is reserved for situations where other treatment options have failed or are not appropriate.
