Formoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Ceylon Leadwort interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with your Foradil or Perforomist (formoterol). A compound in Ceylon Leadwort called plumbagin can slow down a liver enzyme known as CYP2D6, which is one of the pathways that helps clear formoterol from your body. If that enzyme gets blocked, formoterol could linger a bit longer and build up. With a long-acting inhaler like this, that might show up as a faster or pounding heartbeat, jitteriness or shakiness, a headache, or muscle cramps.
The good news is that this concern comes only from lab studies, not from people, so it's theoretical. Still, I'd mention the supplement to your pharmacist or doctor before pairing the two, especially if you already notice your heart racing on formoterol.

Here's the honest picture for your formoterol inhaler. The concern on file is that a compound in Ceylon Leadwort, called plumbagin, may block a liver enzyme known as CYP2C9. When that enzyme is slowed, drugs cleared by it can build up and cause stronger effects. But this is the key point: formoterol is not mainly handled by CYP2C9, so this particular pathway likely matters very little for your breathing medication.
On top of that, the warning comes only from lab and test-tube research, not from real people, so it's theoretical. I wouldn't expect typical formoterol side effects like a racing heart, shakiness, or jitteriness to flare up from this. Still, since Ceylon Leadwort isn't well studied in humans, please run it by your pharmacist or doctor before adding it.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. What Is Formoterol Oral Inhalation Used For?
Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler
Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.
Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
