Why this applies to Trimipramine

Trimipramine falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 2C9 (CYP2C9) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP2C9. Drugs that are metabolized by CYP2C9 include celecoxib (Celebrex), diclofenac (Voltaren), fluvastatin (Lescol), glipizide (Glucotrol), ibuprofen (Advil, Motrin), irbesartan (Avapro), losartan (Cozaar), phenytoin (Dilantin), piroxicam (Feldene), tamoxifen (Nolvadex), tolbutamide (Tolinase), torsemide (Demadex), and S-warfarin (Coumadin).

Here's the gist with trimipramine and Ceylon Leadwort. A compound in the plant called plumbagin can slow down a liver enzyme (CYP2C9) that helps break down certain medicines. If that enzyme is blocked, trimipramine could clear more slowly and build up in your body. Since trimipramine is an active drug (not a prodrug that needs switching on), higher levels usually mean stronger effects and more side effects, like drowsiness, dry mouth, constipation, blurred vision, dizziness on standing, or, at higher levels, heart rhythm changes.

The honest part: this is only seen in lab studies so far, never reported in real people, so it's theoretical. I wouldn't panic, but please run it by your pharmacist or doctor before combining them.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Trimipramine capsules are indicated for the relief of symptoms of depression. Endogenous depression — depression that arises from within, rather than from an obvious outside cause — is more likely to respond to treatment than other types.

In clinical studies, trimipramine worked as well as imipramine in hospitalized patients with depression, and was comparable to amitriptyline in less severely depressed outpatients.
