Prednisolone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down prednisolone. In lab dishes, the plumbagin in Ceylon Leadwort appears to slow that same enzyme down. If that held true in people, prednisolone could clear a little more slowly and build up, which might nudge up steroid side effects like trouble sleeping, mood swings, a bigger appetite, fluid retention, higher blood sugar, or stomach upset.

But the key word here is theoretical. This is based only on test-tube work, and it has never actually been reported in humans, so I wouldn't lose sleep over it. Still, since prednisolone is a serious medication, it's worth a quick chat with your pharmacist or doctor before adding this supplement.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the situation with Ceylon Leadwort and prednisolone. Prednisolone is a corticosteroid you take to calm down the immune system and reduce inflammation, whether that's for arthritis, asthma flares, skin conditions, or preventing transplant rejection. The plant compound in Ceylon Leadwort, called plumbagin, seems to have its own effects on the immune system in lab and animal studies.

The worry here is mostly theoretical. Because both work on immune signaling, mixing them could, in theory, throw off how well your prednisolone does its job, which matters a lot if you're relying on it to control a serious condition or protect a transplant. There's no solid human proof of this, so don't panic. Still, before you add this supplement, please check with your pharmacist or doctor so they can keep an eye on your symptoms.

Prednisolone is prescribed for many different conditions where inflammation or an overactive immune system is causing harm. These range from severe allergic reactions and asthma to certain skin diseases, blood disorders, eye problems, and some cancers.

It is also used to support the body when the adrenal glands aren't making enough of their own steroids, and to manage flares of conditions like ulcerative colitis, multiple sclerosis, and nephrotic syndrome (a kidney condition causing protein loss in the urine).
