Why this applies to Hydrocortisone

Hydrocortisone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist: your body uses a liver enzyme called CYP3A4 to help break down hydrocortisone. In lab dishes, plumbagin (a compound in Ceylon Leadwort) appears to slow that enzyme down. If that happened in real life, hydrocortisone could clear more slowly and build up a bit, which might mean stronger steroid effects, things like fluid retention, higher blood sugar, mood or sleep changes, or stomach upset over time.

I want to be honest about the evidence, though. This is only seen in test-tube studies, never reported in actual people, so it's mostly theoretical. I wouldn't lose sleep over it, but since Ceylon Leadwort can be irritating on its own, please run it by your pharmacist or doctor before combining the two.

Plumbagin, a constituent of Ceylon leadwort, has demonstrated immunosuppressant activity in animal research. Theoretically, Ceylon leadwort might interfere with immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and others.

Here's the gist for hydrocortisone: it's a steroid that calms down your immune system and reduces inflammation, which is why it's used for things like allergic reactions, arthritis flares, and lupus. Ceylon Leadwort contains a compound called plumbagin that, in lab and animal studies, has shown its own immune-dampening effects. The concern is mostly the opposite of weakening your medicine, but honestly, the two together could pull your immune system in unpredictable directions.

I want to be upfront: this is a theoretical interaction based only on test-tube and animal work, not on real people taking both. So I wouldn't lose sleep over it, but I also wouldn't pair them on my own. Please check with your pharmacist or doctor first, especially if your hydrocortisone is treating something serious.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
