Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Ceylon Leadwort interacts with.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 1A2 (CYP1A2) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP1A2. Some substrates of CYP1A2 include clozapine (Clozaril), cyclobenzaprine (Flexeril), fluvoxamine (Luvox), haloperidol (Haldol), imipramine (Tofranil), mexiletine (Mexitil), olanzapine (Zyprexa), pentazocine (Talwin), propranolol (Inderal), tacrine (Cognex), zileuton (Zyflo), zolmitriptan (Zomig), and others.

Here's the thing about Ceylon Leadwort and verapamil: the plant's main compound, plumbagin, has been shown in lab dish studies only to slow down an enzyme called CYP1A2. In theory, blocking that enzyme could let a drug it normally clears build up to higher levels in your body. Verapamil isn't a prodrug, so more of it sticking around would mean a stronger effect, not a weaker one.

With verapamil, that could look like dizziness, unusually low blood pressure, a slow heartbeat, constipation, or swelling in your ankles. I want to stress this is theoretical and has never been reported in actual people, so don't be alarmed. Still, before pairing this supplement with verapamil, please run it by your pharmacist or doctor first.

Plumbagin, a constituent of Ceylon leadwort, has been shown to inhibit cytochrome P450 3A4 (CYP3A4) in vitro. So far, this interaction has not been reported in humans. Theoretically, Ceylon leadwort might increase the levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others.

Here's the gist for Verapamil. Your liver uses an enzyme called CYP3A4 to break this medication down. In lab testing, plumbagin (a compound in Ceylon Leadwort) appears to slow that same enzyme. If it did that in your body too, your verapamil could clear out more slowly and build up higher than expected.

Why that matters: extra verapamil can drop your blood pressure too far or slow your heart rate too much, leaving you dizzy, lightheaded, tired, or constipated. That said, this is theoretical right now, based only on lab studies and never reported in real people. I wouldn't panic, but I would run it past your pharmacist or doctor before combining the two, since heart medications leave little room for surprises.

Ceylon Leadwort (Plumbago zeylanica) is a plant long used in Ayurvedic and other traditional medicine systems, mainly for digestion, skin problems, and pain. Solid human studies are lacking, and the plant contains plumbagin, a compound that can irritate skin and tissues and may be unsafe in larger amounts. Talk to a pharmacist or doctor before using it, and avoid it in pregnancy.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
